Position Title: OB/GYN Team Nurse

Summary: The OB/GYN Team Nurse is responsible for providing care in assessing patient problems and needs; develops and implements plans of care; and maintains clinical documentation. May advise patients on health maintenance and prevention or provide case management per protocol with provider support

Areas of Responsibility:

Patient Care & Education

  • Performs patient care activities that are assigned within the scope of competencies
  • Collaborates with the Care team and Providers to increase understanding, involvement and to enhance the PHC patient experience
  • Performs comprehensive triage and advice (phone and walk-in) for adult/geriatric, young adult, pediatric, infant and pregnant/nursing mothers under the direction of licensed clinical staff
  • Processes medication refills in accordance with protocol
  • Provides immediate response for urgent triage (includes phone) and codes within the facility
  • Performs nurse visits under the direction of Provider (s) (includes, but not limited to): wound care; B12; Coumadin; Testosterone , Progesterone, Methotrexate injections; Insulin, nutrition, and glucometer teaching; Depo; BP checks; suture removal; medication reconciliation; data gathering for walk in pregnant women and patient education for chronic disease per protocol
  • Performs pregnancy testing and counseling for walk in patients
  • Assists with various procedures
  • Performs medication reconciliation and adjustments per protocol and with support of Provider
  • Informs and educates patients on the significance of abnormal lab results relative to chronic disease and/or Public Health tracked illnesses in accordance with PHC approved educational materials as per protocol
  • Effectively utilizes PHC approved educational materials to provide patient education for medications, wellness, disease management, procedures and processes under the direction of licensed clinical staff.
  • Reviews and prioritizes the labs daily for absent providers.

Documentation

  • Ensures a clean, orderly and functional work environment
  • Assists team manager to facilitate policy implementation and adherence by staff
  • Effectively participates in problem identification and resolution
  • Ensures proper care in the use and maintenance of PHC equipment and supplies
  • Promotes continuous improvement of workplace safety and environmental practices
  • Maintains the crash care and checks it on a weekly and monthly basis.
  • Orders progesterone medications and ensures that we have adequate diabetes supplies, the log books and the Sweet Success packets
  • Performs miscellaneous job-related duties as assigned

Case Management

  • Performs indirect case management (includes, but not limited to): patient recalls; missed breast care follow up; and new patient intakes, colposcopy and LEEP follow up.
  • Provides direct case management for uncontrolled chronic disease with Provider direction
  • Maintains ongoing patient status lists in the areas of :Ectopic pregnancy, Spontaneous Abortions, Zika status, Progesterone, pregnant diabetics patients

Training

  • Assists and leads staff trainings
  • Provides guidance to MA’s doing non-appointment work
  • Mentors and orients new staff, students and other team members

Experience and Skills:

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

Education/Experience:

  • Two year degree in Nursing required; Bachelors and/or Master’s degree is preferred
  • Equivalent to an RN or higher degree from an accredited school of nursing
  • One to two years of increasingly responsible nursing experience including experience in community nursing preferred Experience working with the pregnant population preferred but not required.
  • Experience working with the pregnant population preferred but not required.

  • Prepare clear and concise documentation and reports
  • Respond to requests and inquiries from the general public
  • Apply basic time management and patient care coordination skills to effectively provide nursing services
  • Establish and maintain effective working relationships with those contacted in the course of work, including service representatives, Petaluma Valley Hospital staff in the ER , Labor and Delivery, and the general public
  • Plan and organize multiple work tasks, problem-solve and analyze information toward sound decisions
  • Identify problems/issues, develop and evaluate options in order to implement solutions
  • Appropriately manage highly sensitive and confidential patient information

Licenses and Certifications:

  • Possession of an appropriate, valid California Registered Nursing License
  • Possession of, or ability to obtain, an appropriate valid California driver’s license
  • Possession of, or ability to obtain, an appropriate valid CPR certificate
  • Possession of, or ability to obtain, an appropriate valid BLS and ACLS certificate
  • Candidate must be able to successfully meet PHC’s credentialing and privileging requirements

Language Skills: Ability to effectively present information and respond to questions and requests from patients, co-workers, and others as necessary. Bilingual in English and Spanish, both written and verbal preferred.

Mathematical Skills: Ability to add, subtract, multiply, divide in all units of measure, using whole numbers, common fractions and decimals.

Reasoning Ability: Ability to recognize problems, collect data and establish facts

Computer Skills: High level of computer literacy and proficient in MS Office (Word, Excel and Outlook), electronic medical records and web applications. Types 60+ WPM.

  • Movement within health center complex environment repeatedly throughout the day
  • Ability to write by hand, use phone, use computer keyboard and mouse extensively to perform general office functions

  • Ability to effectively communicate by hearing continuously and speech in a manner which can be understood by a diverse population

  • Must have visual acuity near and far (20 inches to 20 feet), depth perception, field of vision, ability to focus on an object, ability to identify and distinguish colors
  • Ability to sit or stand for extended periods of time (minimum periods of one (1) hour at a time)
  • Ability to frequently walk, bend, squat, stoop, kneel, twist, crouch, and climb with the use of a step stool
  • Ability to grasp, hold and pick up and reach with hands and arms
  • Ability to frequently lift and or move up to fifteen (15) pounds
  • Ability to occasionally lift and carry up to thirty-five (35) pounds (periodic lifting of supplies or equipment)
